We are now able to offer Electronic Prescriptions, where your medication request is sent directly to your nominated pharmacy without the need to visit the surgery to collect your prescription form.

You can nominate any pharmacy to receive your prescription simply by visiting that pharmacy and asking for a nomination form.

Dr Cruz Augustine (m)


Dr AugustineMBBS, MRCGP, MRCP - Dr Augustine graduated in the year 2000 from Madras, India and did his hospital training at Blackpool Victoria Hospital since 2002 in specialities including medicine, paediatrics and emergency medicine. He joined the practice in 2007 and has a diverse medical interest with special interests in cardiac disease, diabetes, epilepsy, child health, family planning and minor surgery. He is a trainer and mentor for trainee doctors.

Dr Prasanna Anthony (m)

MBBS (SL), MCRP (UK), MRCP (Edin), MRCPS, MRCGP Dr Anthony
- Originating from Sri Lanka, Dr Anthony has been associated with the practice since August 2011. He completed his GP training at Arnold Medical Centre in Blackpool and Fernbank Surgery in Freckleton.

Dr Joydip Majumder (m)

Dr Majumder joined the practice in May 2015 and has interests in dermDr Majumderatology, ENT, Musculoskeletal medicine and Minor surgery. He does a wide range of joint injections too and is passionate in research in General Practice. Joydip is a accredited GP Trainer, Appraiser, LMC Mentor and has various undergraduate supervisory roles at Manchester University.

Qualifications: BSc (St. Andrews), MBCHB (Manchester) MRCS, MRCGP, diploma of GP education (UCLAN).

Dr Tina Mistry (f)

MBChB (2008), MRCGP (2013), DFSRH (2016), DRCOG (2016)

Dr MistryDr Mistry qualified from the University of Liverpool. She has worked at the East Lancs hospital trust during her foundation training and then at Pennine Acute hospital trust during her GP training. She now works as a locum GP in the Blackpool and Fylde coast area. She has a wide variety of interests but particularly enjoys palliative care and has worked in hospice settings previously. She is planning to do the family planning diploma over the coming year.
